Terms of service.
Product & service pricing
Katie Cox Nutrition reserves the right to alter prices for products and/or services offered. Prices are final and alterations to prices are at the discretion of the primary practitioner, Katherine Cox, only. Prices listed on the website are considered the most updated prices and will be enforced.
Prices for products from third parties are not under the control of any practitioner at Katie Cox Nutrition. This includes the prices of prescription items such as nutraceutical supplements, functional and pathological testing and other products/services purchased through third party suppliers.
Payment terms
A 50% non-refundable deposit will be payable at the time of booking. Bookings made online can be paid through our secure online portal. Should the deposit not be received, the booking is not considered complete and no practitioner of Katie Cox Nutrition is held accountable or is required to provide a service before payment is received.
Bookings made via phone will require credit card details in order to complete a payment before finalising and completing a booking for a service and/or product.
Clients may choose to pay in advance for future consultations at the time of booking. Payments can be made via direct deposit, payid or via EFTPOS.
Products need to be paid in full at time of consultation and/or ordering.
Payment plans and discounts are not offered at this time.
Bookings and form completion
Once a booking for a consultation is made, a number of documents will be forwarded for completion and signature. Please note that the consultation cannot commence without these documents being completed and forwarded back to Katie Cox Nutrition.
Forms to complete typically include (but are not limited to) the Client intake form, personal and medical history details, diet and symptom recall diary, statement of acknowledgment and consent, coronavirus statement of declaration.
Katie may also request copies of previous pathology and/or specialist test results prior to your consultation.
Completion of all these forms in advance assists us in providing you with better service and allows the full consultation time to be used for consultation purposes.
We reserve the right to refuse services or cancel appointments should these conditions not be met.
Extenuating circumstances may apply, which will be at the discretion of the practitioner.
Cancellations & refunds
Understandably, things happen that are out of our control. Katie Cox Nutrition will strive to accomodate all clients where possible, at the discretion of the primary practitioner.
Cancellations within 24 - 12 hours prior to consultation will incur a charge of 50% of the original consultation fee.
"No show” or "on the day" cancellations will receive no refund. 100% of the original fee associated with your nominated service will be retained.
Rescheduling an appointment more than one (1) time consecutively will incur a cancellation fee charge.
Extenuating circumstances may apply. These will be at the discretion of Katie Cox. Supporting evidence may be required. All decisions are final.
Prescription products & testing services
Third party dispensary services are used. While Katie Cox Nutrition strives to only use the best and provide products of the best quality, we do not claim any responsibility for the products / services received from any third party. Should you be dissatisfied with any products received, we will do our best to rectify the situation but cannot provide refunds ourselves. This is up to the third party supplier.
Shipping costs, times and manners are not the responsibility or under the control of Katie Cox Nutrition.
Prescription products and testing service costs are additional to consultation and booking fees for Katie Cox Nutrition and is not collected or kept by Katie Cox.
Before prescriptions can be processed and sent to your nominated postal address, full payment must be received by the third party. Before results can be released to Katie Cox Nutrition by any testing service party, full payment of the service will be required from you and processed by the nominated third party.
Minors (under 18 years of age)
While Katie Cox Nutrition practitioners are able to treat and assist a person of any age, including infants, any persons under the age of 18 years will require a consenting adult / guardian present.
A consent to treat form must be completed and signed before any practitioner can assist any person/s under the age of 18 years.
Business details & structure
Katie Cox Nutrition is an entity operating as a sole trader run by Katherine Cox. Katherine Cox is a qualified Nutritionist, holding a Bachelor of Health Sciences (Nutritional Medicine & Dietetics). Katherine Cox is also a registered member with the Australian Natural Therapists Association (member no 23025).
